How can Guest / Substitute Players Submit their
Player Waiver & Release Form

Each guest / substitute player must complete the Player Waiver & Release Form before the team's first game.  The waiver form can be found here: http://waiver.fxafootball.com

Once this online waiver is completed, you will never have to complete another Fairfax Athletics waiver again unless your personal information changes (Address, Email Address, Cell Phone #, etc).  This waiver will then be archived in our system and will be used for all Fairfax Athletics sports and social events. 

*Please Note - If a player uses multiple email addresses (work & personal), they need to enter all email addresses in the email field, seperated by a comma.
